An ADHD sick note in the UK may be considered when attention deficit hyperactivity disorder, a related mental health problem, treatment effects or another assessed condition makes you temporarily unfit for work. ADHD does not automatically mean you need time off. Many people remain at work with the right structure, communication and reasonable adjustments.

This guide explains when sickness absence may be appropriate, what a clinician assesses, how self-certification works, and how a fit note can support a may-be-fit conversation. It also covers Equality Act protection and Access to Work without assuming that every person with ADHD has the same symptoms or legal status.


Can You Get a Sick Note for ADHD?

A healthcare professional may issue a fit note if your health affects your fitness for work. The decision is based on current function, not the diagnosis label alone. They may consider concentration, impulsivity, sleep, emotional regulation, anxiety, depression, medication changes and whether your job involves driving, machinery, safeguarding, financial decisions or other safety-critical tasks.

A fit note can say not fit for work when you cannot safely do any work for a period. It can instead say may be fit for work and suggest support such as altered hours, amended duties, workplace adaptations or a phased return. The employer decides whether it can implement the suggestions, ideally after discussing them with you.

ADHD symptoms can fluctuate with workload, sleep, hormones, stress, physical illness and access to treatment. A temporary crisis does not necessarily mean the underlying condition has permanently worsened. The assessment should separate short-term incapacity from longer-term barriers that could be addressed at work.

The NHS guide to ADHD in adults advises speaking to a GP when symptoms affect work, studies or relationships. It also notes that medicines must be started and monitored by an ADHD specialist. A sick-note assessment is not an ADHD diagnostic assessment and should not be used to start, stop or change prescribed medication without the responsible clinician.

When ADHD Symptoms May Affect Fitness for Work

ADHD can involve difficulties with attention, organisation, working memory, restlessness and impulse control. The impact depends on the person and the job. Missing a routine email in one role is different from losing track of a safety sequence in a clinical, transport, construction or industrial setting.

Some people seek help after a period of escalating workload, repeated sleep loss or burnout. Others are affected by an interruption to treatment, adverse effects during careful medication titration, or coexisting anxiety or depression. The clinician needs a clear timeline and should consider whether urgent treatment review is more important than documentation.

  • Severe sleep disruption that makes concentration or driving unsafe.
  • Marked anxiety, low mood, panic or burnout alongside ADHD symptoms.
  • A treatment change causing significant side effects that require clinical review.
  • Repeated near misses or loss of safe task control in a high-risk role.
  • Inability to organise essential daily activities despite usual strategies and support.
  • A workplace crisis that cannot be stabilised immediately with temporary adjustments.

Ordinary distraction, a difficult deadline or dissatisfaction at work does not automatically establish medical incapacity. Explain what you cannot currently do, what happens when you try, and why that creates a health or safety problem. Functional examples are more useful than saying only that ADHD is bad.

If symptoms are mainly caused by an unsupported workplace, sickness absence may provide short-term breathing space but will not fix the barrier. Self-Certification, Fit Notes and Sick Pay

Employees normally self-certify for the first seven calendar days of sickness absence. You must still follow the employer's notification procedure, including its deadline and contact method. Calendar days include weekends and days you do not normally work.

After more than seven days, an employer may ask for a fit note or other medical evidence. A fit note can be issued after an appropriate assessment in person, by telephone or online where suitable. Government guidance says the clinician should consider the effect of the health condition on work and whether some work could be possible with support.

If an employer requests medical evidence during the first seven days, it should pay any fee charged for that evidence. Clarify whether it needs a statutory fit note, a private certificate or an occupational-health report. These documents have different purposes, and a private certificate is not automatically interchangeable with the Med3 used for statutory systems.

From 6 April 2026, eligible employees can receive Statutory Sick Pay from the first full qualifying day. The amount is 80% of average weekly earnings or the current statutory flat rate, whichever is lower, and the previous lower earnings limit no longer applies. ADHD, Disability and UK Employment Rights

ADHD can amount to a disability under the Equality Act 2010 if it has a substantial and long-term negative effect on normal day-to-day activities. The word substantial means more than minor or trivial, and long term generally means the effect has lasted or is likely to last at least 12 months. The legal test looks at impact, not simply whether a diagnosis exists.

An employer has a duty to make reasonable adjustments when it knows, or could reasonably be expected to know, that a worker is disabled and disadvantaged. What is reasonable depends on effectiveness, practicality, cost, resources and the nature of the role. Not every requested change must be accepted, but the employer should consider the request properly.

Acas states that a worker does not need a diagnosis before an employer considers neurodiversity adjustments. It recommends collaboration, trial periods and review. Its guidance on adjustments for neurodiversity includes practical examples for concentration, communication and task planning.

You choose how much medical information to share, but the employer needs enough information to understand the disadvantage and consider support. A fit note can contribute to that conversation but is not the only route. A written request, occupational-health advice or an Access to Work assessment may provide more detail for ongoing changes.

Disability protection does not make every absence immune from normal management. Employers can manage attendance and performance, but they must avoid discrimination, consider adjustments and assess disability-related absence fairly. Get advice from Acas, a trade union or an employment adviser if the process seems inconsistent or punitive.

Reasonable Adjustments for ADHD at Work

The most useful adjustment addresses a specific barrier. Start with the task or environment that is difficult, then propose a change and a review date. A large list copied from the internet is less persuasive than two or three focused adjustments tied to your role.

  • Concentration: a quieter desk, permission to use noise-reducing headphones, protected focus time or fewer unplanned interruptions.
  • Instructions: short written steps, clear priorities, examples of the finished task and confirmation of changed deadlines.
  • Planning: regular brief check-ins, task chunking, shared calendars or agreed reminder tools.
  • Time: flexible start times, structured breaks or protected time for complex work where operationally possible.
  • Meetings: agendas in advance, written actions afterwards and permission to move or use a fidget aid without disruption.
  • Return to work: a temporary reduction in workload, amended duties or phased hours with scheduled review.

The NHS suggests a personalised quiet workspace, written as well as spoken instructions, and help structuring tasks. Acas gives similar examples but stresses individual discussion. Some people work better at home, while others need the external structure of a workplace. Avoid assuming that one arrangement suits everyone with ADHD.

Access to Work can provide practical support beyond the adjustments an employer is legally responsible for. GOV.UK specifically lists ADHD as a condition that may qualify and says a formal diagnosis is not always required to apply. Support may include coaching, equipment or mental health help, subject to assessment and location rules.

Preparing for a Clinical Assessment

A focused assessment is easier when you prepare specific information. Write down what changed, when it changed and what the consequences have been. Include your job demands, normal strategies, adjustments already tried and any recent treatment or sleep changes.

  • Your diagnosis status and which clinician manages ADHD treatment.
  • Current prescribed medicines, recent changes and any suspected side effects.
  • Sleep pattern, anxiety, mood, appetite and substance or alcohol use where relevant.
  • Examples of impaired function or safety concerns, without sharing confidential employer data.
  • Existing adjustments and why they are no longer enough.
  • What work you might still manage with temporary changes.

Do not stop ADHD medicine suddenly or change the dose solely to get through work. Contact the prescriber or an appropriate clinician if you have side effects, supply problems or concerns. Seek urgent help for severe mental health symptoms, chest pain, collapse, thoughts of self-harm or an immediate safety risk.

A clinician may decide that you are fit for work, may be fit with support, need a short absence, or need a different assessment. They may advise you to contact your GP, ADHD specialist or emergency care. Paying for an assessment does not guarantee that a certificate will be issued.

Returning to Work After ADHD-Related Absence

A return plan should address the trigger for absence. If workload, interruptions and unclear priorities contributed, resuming the same conditions at full intensity may lead to another crisis. Agree a small number of temporary changes before the first day back and schedule a review.

Keep a brief record of agreed adjustments and how they work. Review effectiveness rather than judging effort or personality. If a quiet area improves accuracy but a flexible start creates handover problems, keep the helpful change and revise the other one. Reasonable adjustments are often an iterative process.

A return-to-work meeting can cover confidentiality, workload, medication appointments, emergency contacts and early warning signs. You do not need to promise that symptoms will disappear. The goal is safe, sustainable work with clear expectations and a route to ask for help.
